The PATH project

Welcome to the Partnerships for Access to Health (PATH) Project website.

PATH aimed to explore how Community Health Partnerships (CHPs) can work with local social care, community justice and voluntary sector partners to improve access to services for people with multiple, complex health and social care needs.
